I'm sending prayers of thanks that there was no tragedy. Besides the personal tragedy, this was almost a disaster for shooting sports in general. Is there a formal investigation into how this happened? I think we'd all like to hear suggestions of how to better avoid accidents in the future.

No investigation needed. The Safety officer/ RSO has the final say before loading and making ready. He screwed up, for a damn long while, then by accident nothing happened. See thru walls are a good thing.
